Step-by-Step: How to Collect Feedback

Step 1: Share Your QR Code

• You’ll receive a QR code linked to your event feedback form.

• The Event link form is also below.

• You can print the code, display it on a sign, or show it on your phone or tablet.

• Ask participants to scan the code using their phone camera to access the short survey.

Pro Tip: Encourage participants to complete the form during your event (e.g., after a group discussion, during a snack break, or before leaving the event).

Step 2: Let People Know Why It Matters

When inviting attendees to complete the survey, you can say:

“We’d love your feedback to help us improve future events and better support our community. It only takes 2–3 minutes, and your voice makes a difference.”

 

Step 3: Help People Complete the Survey (if needed)

• If someone doesn’t have a smartphone, you can help them by pulling the form up on your device and reading the questions aloud.

• Encourage honesty, and let people know why their candor is crucial to this work.

Step 4: Thank Them and Offer Support

After someone completes the survey, thank them and offer to answer any questions they might have about the information you shared.
